Fixing it properly means dealing with the cause first, then removing all the loose paint, preparing the surface, priming and repainting. If you only paint over the flakes, the new coat will peel along with the old one, often within months.
There’s one important safety question before you start: how old is the paint? Lead was widely used in household paint for decades. In the US it was banned for homes in 1978, and many other countries phased it out at different times, with some decorative paints containing lead much later. Check the age of the paint and test for lead
If the home was built before 1978 in the US, or is an older property elsewhere, use a lead test kit on the peeling layers, or hire an inspector. Test all layers, not just the top coat. If lead is found, either follow lead-safe work practices (wet methods, containment, careful cleanup) or hire a lead-safe certified contractor.
Warning: Lead dust is invisible and settles on floors and window sills where children play. Don’t start scraping until you know.

Find and fix the moisture source
Look at where the peeling is. In bathrooms and kitchens, poor ventilation is the usual culprit, so make sure the exhaust fan works and runs long enough. On ceilings, look for a leak above. On exterior walls or low down on ground-floor walls, suspect rain penetration, a leaking gutter, or rising damp. Fix the cause, then let the wall dry fully.
Tip: A moisture meter helps confirm the wall is dry before you paint. Aim for a reading similar to a dry wall elsewhere in the room.

Protect the room
Move furniture out or to the middle of the room and cover it. Lay plastic sheeting on the floor and tape it at the wall. Close doors and turn off any forced-air heating or cooling in that room so dust isn’t spread.

Scrape off all loose paint
Hold the scraper at a low angle and push under the loose edges. Remove everything that lifts easily, and keep going until you reach paint that is firmly stuck. For lead-safe work, lightly mist the surface with water before scraping to keep dust down, and let chips fall onto the plastic.
Warning: Don’t gouge the plaster or drywall paper. If the surface underneath is crumbling, it will need repair before painting.

Treat efflorescence on masonry
If you see white powdery salts, brush them off with a stiff dry brush. Don’t wash them with lots of water, which just dissolves more salt. The underlying damp must be solved or the salts will keep coming back. A masonry primer or sealer designed for alkaline surfaces helps once the wall is dry.

Feather the edges and fill
The edges where old paint remains will show through new paint. Sand them smooth with a fine sanding block (wet sanding only if lead is present), or skim them with a thin coat of filler. Fill dents and cracks, let the filler dry, and sand smooth.
